The endurance shuttle walk test is done after the incremental shuttle test. You walk around the same ten-metre course, but this time at one steady, set speed, for as long as you comfortably can. Because it holds the effort constant, it is especially good at showing whether rehabilitation, oxygen or medication has improved your stamina.
What it measures
The endurance shuttle walk test (ESWT) is a constant-intensity, externally paced field test of endurance capacity. The patient walks at a fixed speed — usually set at about 85% of the peak reached on the incremental shuttle walk test — and the outcome is how long that pace can be sustained.
How it is done
After a short warm-up level, the patient walks around the 10-metre course in time with the audio signals at the pre-set speed until they can no longer keep up or must stop for symptoms.1 Endurance time (in seconds), and sometimes distance, is recorded along with SpO₂, heart rate and Borg scores.
Interpretation
- The primary result is endurance walking time.
- The minimal important difference is in the region of 60–85 seconds (around 180–190 m).
- The ESWT is more responsive to change than distance-based tests, which makes it valuable for evaluating treatment.
Use in cardiorespiratory physiotherapy
It is a preferred outcome measure for pulmonary rehabilitation and for assessing the effect of interventions such as ambulatory oxygen, non-invasive ventilation during exercise, or bronchodilators. It must be preceded by an ISWT to set the walking speed.2
Cautions
An incremental shuttle walk test is required first to establish the pace. The same cardiac precautions and stopping criteria apply as for the other field walking tests, and the course and pace must be kept identical between tests.3
Prescribing exercise from the result
The ESWT output is a time, and it becomes the training target directly: walk the patient at ESWT speed and progress toward, then beyond, the endurance time achieved. Being the most responsive of the field tests to rehabilitation, it is also the best of them for demonstrating that a programme has worked. See prescribing exercise from test results.
