Lifestyle Changes Programme
A physiotherapy-led programme of supervised exercise and practical education, aimed at building strength, protecting muscle and improving how you function day to day.
When people make significant changes to their weight or activity, muscle is often lost alongside fat. Structured exercise is what protects it. This programme is built around that — keeping your strength and physical function moving in the right direction while the rest of your health goals are being worked on.
It begins with a full cardiorespiratory and functional assessment, so your plan reflects your starting point, your goals and any precautions that apply to you.
What’s involved
Establishing your baseline, your goals and any exercise precautions that apply to you.
Progressed at a pace that suits you, with your physiotherapist alongside.
Pacing, breathing, sleep, and building routines that hold once the programme finishes.
Progress checked as you go, with a reassessment at the end of the programme.
What the programme is aimed at
Each part of the programme is built around keeping your strength and physical function moving in the right direction.
Muscle strength
Maintaining and building strength, which is often lost when weight or activity levels change.
Everyday function
Improving exercise tolerance and how you manage the physical demands of an ordinary day.
Cardiometabolic health
Supporting heart, lung and metabolic health through regular, structured activity.
Habits that hold
Establishing activity routines that continue once the programme finishes.
Where appropriate, and with your consent, the programme is coordinated with your general practitioner (GP) and wider medical team.
Any medical aspect of your care remains a matter for your GP or treating specialist physician. With your consent, we work alongside them and share your progress. Physiotherapists do not prescribe medicines.
Results vary from person to person. Your physiotherapist will talk through what is realistic for you at your assessment.
Who it's for
The programme suits people who know roughly what they should be doing and have not been able to make it stick, and people whose starting point feels too low to walk into a gym. It is built for adults living with or at risk of heart and lung conditions, raised blood pressure, diabetes, or the deconditioning that often travels with them.
You do not need to be fit to start, and you do not need to have succeeded at this before. Most people who come to us have tried something already; the programme is designed around that being normal rather than a failure.
How the programme runs
You start with an assessment, and the programme is built from what it finds rather than from a template. Sessions usually run weekly or fortnightly across eight to twelve weeks, each combining supervised exercise with a short, practical conversation about one change — sleep, activity through the day, meal timing, medication routine — rather than a lecture covering everything at once.
Between appointments you have a home programme deliberately small enough to still happen on a bad week. Progress is recorded with the same measures each time: strength, exercise tolerance, blood pressure where relevant, and your own account of what you can do now that you could not do before.
